The following questions are an opportunity to share where you are on your Discipleship Journey. The questions are personal and require some thought; we encourage you to take your time and be thorough in your responses. Please do not fill out this form for any one else. If you have a family member who is also interested in membership, please have them fill out their own copy. Use this as an opportunity to reflect on all God has done (and is doing) in and through you since you surrendered your life to the Lordship of Jesus Christ.
